Financial and Insurance Information

Our office encourages payment at the time of service, regardless of insurance coverage.

We consider the patient primarily responsible for the account, but we are happy to submit claims to your primary insurance carrier on your behalf. Please help us to keep your insurance records up to date and accurate so that reimbursement can take place in a timely manner. If you are covered by a managed care insurance plan, co-payments are expected at the time of service.

Some insurance plans pay for preventive/wellness care while others, including standard Medicare, do not. We ask that you check your insurance benefits to know whether or not there is coverage for preventive care. If your insurance does not cover preventive care, and if you obtain this service, you will be directly responsible for the portion of the visit charge that pertains to preventive care. If your insurance does cover a preventive examination, and at the time of such an exam there are significant illnesses addressed, your insurance will be billed for both a preventive and illness visit.

Please notify your physician at the time of your visit if you have coverage for, and are expecting, a preventive examination.
